Required Evaluation Questions

Grant projects that benefit the library workforce or the general public will be evaluated using outcome-based assessment questionnaires (surveys) when they include: 

  • Program instruction 
  • Content acquisition or creation 
  • Planning and evaluation 

We provide a quick reference guide to help you determine when to survey LSTA project participants, and four examples of the required outcome-based questionnaires.  

These examples of required questions are the core areas that need to be covered. However, we encourage you to ask any additional questions you believe will help measure the program’s success. 

You will be required to provide documentation proving these questions were incorporated into your evaluation process.
