Required evaluation questions

Grant projects which benefit the library workforce or the general public should be evaluated using outcome-based assessment questionnaires (surveys) if they include:

  • Program instruction
  • Content acquisition or creation, or
  • Planning and evaluation

We have provided a quick reference guide to help you determine when to survey LSTA project participants, and four examples of the required outcome-based questionnaires. 

These examples of required questions are the core areas that need to be covered. However, you are free to ask any additional questions you feel will help measure the program’s success.

You will be required to provide documentation indicating incorporation of these questions into your evaluation process.  

Mid-Year Report

A Mid-Year Report for project accomplishments, expenditures and use of evaluative data is due January 30, 2021.

Final Report

A Final Report for the project is due November 1, 2021. This report provides accomplishments, an evaluation of the impact of the project and a financial accounting of expenditures.

During FY2021-22, the percentage of total costs for the Administration of the Annual Program financed with federal money is 66%; the federal dollar amount to be spent on the program is $431,996. This program does not receive any nongovernmental funding.
